1. Individual must be an Active Member of the Academy in good standing and a Certified Creighton Model FertilityCare™ System Practitioner or Instructor.

2. Must have a current practice in the Creighton Model FertilityCare™ System, i.e. has taught at least 10 couples in the past year.

3. Individual has made an outstanding contribution to the Creighton Model FertilityCare™ System through service, education, and/or research.
	Developed and/or administrates an accredited Creighton Model FertilityCare™ service program Developed and/or administrates an accredited Creighton Model FertilityCare™ education program Is the principal investigator of practice based NFP research that has been published in a recognized professional journal Maintains a large Creighton Model FertilityCare™ practice under adverse conditions
